Job Description

Title: Physical health liaison worker

Hours:

2 x 35 hours full time

1 x 21 hours part time

Salary: ยฃ28,000

Contract: 12 month fixed term

Location: TBC โ€“ Across North West Surrey and Guildford and Waverley

About the role

People living with severe mental illness (SMI) face one of the greatest health inequality gaps in England, dying approximately 20 years earlier than the general population in most part due to preventable physical health diseases.

This new initiative aims to reduce this inequality by supporting the uptake of physical health checks that people with a diagnosed severe mental health illness are entitled to but do not always access.

As a Physical Health Liaison Worker you will work holistically, directly with individuals with a SMI to increase the uptake of physical health checks and create positive lifestyle changes, in partnership with other key health and social care professionals in the Surrey Heartlands area.
